best life in their own home and in their community.

Positions Available

Community Living Connections is a Local Non-Profit and provides support throughout Dane County. Individuals are supported in the communities of Madison and Stoughton.

Because we believe every individual can make a unique contribution to our society, the majority of individuals we support are employed in the community and are out of their homes during the day. Typical work shifts include mornings, afternoon/evening hours and weekends. Overnight support is also utilized in some households and can be either awake or sleep.

Seeking Self-Starters for an Employee Liason role!

The Scheduler will work under the direct supervision of the Division Manager and will work cooperatively with the scheduling team and other Mangers to ensure the staffing needs of CLC are met. 

This is an excellent opportunity for a candidate who is a self starter, who  works well independently and has the ability to identify issues and develop solutions that meet the needs of the people we support and the people who work at CLC. 

Responsibilities

  • Maintaining and monitoring household schedules
  • Proactively communicating with employees to ensure all shifts are filled 
  • Liaison for employees with schedule changes and time off requests
  • Assist in identifying schedule gaps due to anticipated, or emergency absences
  • Scheduler will work Tuesdays through Fridays and every other Weekend (Saturday and Sunday)
  • Position includes a minimum of 8 hours of in-home Direct Care

Position Requirements

  • Effective communicator - in writing and verbally
  • Proficient using phones, computers and Microsoft Office.
  • Must be 18 years or older to apply

Community Support Managers (CSMs) provide high-quality case management services to adults with developmental disabilities who live in their own home in the community.

CSMs are responsible for supervising and training staff, for coordinating community services for people we support, maintaining written documentation, regular communication, schedule and attend appointments, and quality assurance.

As a Manager, you'll work 20-30 hours per week in the homes of the people we support. This allows you to better build relationships with them, their family members, the staff who work directly with them and other members of their support team! You'll get to know their personal aspirations, and help them reach those goals! As a CSM you will also be providing supports to help them live their best life, which includes assuring their health and safety needs are met. Each individual has different support needs which may include assistance with personal cares, helping them learn how to cook or maintain their home, or attending doctor's appointments together. CSM’s play a crucial role in helping individuals to make connections and build meaningful relationships within their community.

Responsibilities Include

  • Supervise, support and train direct care staff
  • Identify, coordinate and monitor community services for individuals on caseload
  • Quality assurance and compliance with policies and procedures
  • Maintain records
  • Regular communication with service providers, family members and guardians, and support team
  • Coordinate necessary medical services and equipment, may include attending appointments
  • 20-30 hours per week working in homes

Position Requirements

  • Must be 18 years or older to apply
  • Strong supervisory and leadership skills
  • Ability to routinely make sound decisions regarding complex issues
  • Solid work history
  • Prior experience working with adults with developmental disabilities in a residential setting
