Premium: Single: $64.80/paycheck. Family coverage: $260.98/paycheck. When two individuals who are married and employed Full-Time at CLC, their premium is $0!
Copay: $20/Primary visit, $40/Specialist visit, $40/Urgent Care visit
Delta Dental Insurance: $50 deductible for Single, or $150 deductible for Family.
Premium: Single coverage: $2.87. Employee and children: $11.05. Employee and spouse: $8.70. Family: $17.36.
A few highlights: 100% of Diagnostic and Preventative services are covered.
Delta Dental Vision Plan:
Premium: Single coverage: $3.09. Employee and children: $6.30. Employee and spouse: $6.18. Family: $9.39.
Copay: $10 copay in Network, covers comprehensive spectacle exam.
A few highlights: $150 frame/contact allowance. 15% off retail price of Laser Vision Correction.
Additional Benefits for Full Time Employees
Flexible Spending Plan, a plan set up by the IRS that allows you to save money, through pre-tax payroll deductions for health and dental insurance premiums, unreimbursed medical expenses, & dependent day care expenses.
Life Insurance, Long Term Disability Insurance, & Short Term Disability Insurance: CLC pays 100% of the cost. Full Time employees are automatically enrolled.
