    • Preventive services: no charge

    • Physician visit: $20

    • Special visit or Urgent Care visit: $40

Delta Dental Insurance

  • $50 deductible for single or $150 deductible for family

  • Premium:

    • Single coverage: $2.87/paycheck

    • Employee and spouse: $8.70/paycheck

    • Employee and child(ren): $11.05/paycheck

    • Family: $17.36/paycheck

  • 100% of diagnostic and preventative services are covered

Voluntary Vision Plan - Delta Dental/EyeMed

  • Premium:

    • Single coverage: $3.09/paycheck

    • Employee and spouse: $6.18/paycheck

    • Employee and child(ren): $6.30/paycheck

    • Family: $9.39/paycheck

  • $10 in-network copay annual eye exam

  • Frames allowance: $150, then 20% off balance

  • Contacts allowance: $150, then 15% off balance

Additional Benefits for Full Time Employees

Flexible Spending Plan, a plan set up by the IRS that allows you to save money, through pre-tax payroll deductions for health and dental insurance premiums, unreimbursed medical expenses, & dependent day care expenses.

Life Insurance, Long-Term Disability Insurance & Short-Term Disability Insurance: CLC pays 100% of the cost. Full-time employees are automatically enrolled.

Employee assistance program: Full-time employees have automatic free access to these confidential services. Including travel assistance, bereavement support, legal & financial consultation (e.g. will preparation).

401(k) Plan: 4% company match after 2 years of employment. Employees can enter the plan at the beginning of any quarter.
